What is your new patient procedure?
After requesting a screening or evaluation, patients will be sent intake paperwork via email. Paper forms will be provided upon request. New patients will need to fill out intake forms before arriving to their first appointment.
Do you accept insurance?
We currently accept Blue Cross Blue Shield of Michigan, as well as private pay. This includes use of HSA and Flex Spending Accounts.
How do I access your office?
Our office is located within the Center for Well-Being on Shiawassee Ave (A.K.A. "Big Blue").
Parking is behind the building and can be accessed off Adelaide.
Enter in at the main entrance in the rear of the house, go to your left toward the main house, and I'm located on the second floor, 2nd door on the right.
